HARRIS COUNTY, Texas (KTRK) -- A report into a Tesla crash that left a 76-year-old woman dead in the Katy area last month has just been released.

The video above is from a previous report: Crash victim's family files lawsuit against Tesla, driver On Wednesday, the National Transportation Safety Board (NTSB) released a preliminary report regarding the June 19 incident.

Michael Butler, identified as the Tesla driver, is charged with manslaughter after he allegedly crashed into a home on Rose Hollow Lane, killing 76-year-old Martha Avila.
