SMITH COUNTY, Texas (KLTV) - A DPS crash report lists "fatigued or asleep" as a factor in an Uber driver's crash that killed one of her passengers in July.

Chelsea Breanne Dillard, 34, died in the crash on FM 14, north of Tyler Loop 323, on July 23.

According to the DPS report, the Uber driver was driving a 2025 Nissan Sentra north on FM 14 at 6:18 a.m. She crossed the double yellow line into the southbound lane and drove into a ditch and struck a fence.
